Please send us the photo(s) of your loved one. Also provide the full name, date of birth and date of death. If you’d like to include their place of birth (state) indicate this as well when submitting your form.
Buyer should know : A draft of your Prayer ID Card will be sent to you before your purchase. At least two drafts will be provided. After you approve the selected draft, you will be expected to make the payment. Your bookmarker will be shipped 48 hours after approval. An email will be provided with a payment link and confirmation about the shipment and when you can expect delivery.
